All information provided on the Platform and as part of the Platform Services is for informational purposes only and should not be considered a medical advice, diagnosis, or treatment, nor should it be considered a substitute for any clinical support system that may exist in a healthcare setting. We advise that you consult with a qualified healthcare professional to validate any information or any perceived recommendations provided as part of the Platform Services or sent to you through email by or on behalf of Cornie Health. For questions or consultations regarding your healthcare, please call or see your physician or other qualified healthcare provider.
These terms of use shall govern your access or use of the Cornie Health platform through her mobile and web application (collectively, the Platform). The Platform provides access to healthcare services, practice solutions, benefits solutions, and technical and administrative support. These Terms of Use are a legal contract between Cornie Health Technologies Limited and you. For the purpose of this contract, "you" refers to the person or entity using the Platform or receiving any Platform Services. You may download a copy of these Terms of Use, save and print for your reference. By accepting the Terms electronically or otherwise, you agree to be bound by these Terms of Use. If you do not wish to accept and agree to be bound by these Terms of Use, please do not use the Platform.
Cornie Health may, at her discretion, amend any part of these Terms of Use at any time by giving 30 days notice. Notices regarding modifications to these Terms of Use will be in a written form and provided to you: (i) via the Platform; or (ii) via email (to the email address associated with your Cornie Health account). You are not obliged to continue using the Platform Services after amendment(s). If the new Terms are not agreeable to you and you choose not to accept the changes, you must immediately cease to use the Platform and the Platform Services. If you continue to use the Platform Services following any amendment, it shall imply your acceptance of the amended Terms of Use, and waiver of any additional notice requirements. Cornie Health reserves the right to change any information, material or content contained on or provided through the Platform (the "Content") at any time, and from time to time, without notice. Content does not include your Personal Information.
Use of the Platform
These Terms form a legally binding contract between you and Cornie Health. By directly accessing the Platform, you represent, acknowledge, and agree that you are at least 18 years of age; the Platform is not for you if you are under 18. Cornie Health may terminate these Terms and your use of the Platform if Cornie Health, in its sole discretion, believes that you are under the age of 18.
Cornie Health grants you a personal, non-transferable, non-exclusive, and revocable license during the Term to permit you to use the Platform, including the right to download and install the App on your mobile phone device to access the Platform Services following these Terms of Use.
As a condition of your use of the Platform, you warrant that: (1) you are at least 18 years of age; (2) you possess the legal authority to create a binding legal obligation; (3) you will use the Platform following these Terms of Use; (4) all information supplied by you on the Platform is true, current, accurate, and complete; and (5) if you are accessing or using the Platform on behalf of a third party (another person or a corporate entity), you represent and warrant that you have the authority to bind such party to these Terms of Use.
Cornie Health-affiliated practices are independently owned and operated by licensed practitioners who provide services utilizing the Cornie Health platform.
